Requirements For Students Entering 7th Grade
- Current physical exam (Please be sure their physician checks their hearing, vision, height and weight during the exam).
- Tdap (Tetanus, Diphtheria, and Pertussis) vaccination
- Meningococcal vaccination
- *Verification of these immunizations must be received in the Health Office before entrance to Grade 7.
- All students trying out for sports must take the "Concussion and Sportsmanship Course" yearly
- A parent must fill out the Athletic Forms, you can find them under the "Downloadable Documents" tab.
- Student's must have a current physical exam (withing 13 months of tryout) to participate in tryout.
- ALL prescription and over the counter medication needs a physician order and parent consent. Each medication must have a completed Medication Order form. You can find these forms in the "Downloadable Documents" tab.
- ALL medication must be delivered by a responsible adult, in the original labeled prescription bottle or box.
- Medication Orders must be renewed every school year.
EPI-PEN INFORMATION:
- There are several forms that must be completed for a student to have an Epi-Pen in school
- Forms must be fully filled out and signed by the parent and physician.
End of Year Medication Information
- ALL MEDICATION MUST BE PICKED UP BY A PARENT/GUARDIAN BY THE LAST DAY OF SCHOOL. ANY MEDICATION NOT PICKED UP BY THIS DATE WILL BE DISPOSED OF.
- ALL MEDICATIONS NEEDED FOR NEXT SCHOOL YEAR NEEDS A NEW MEDICATION ORDER FORM.
